What Services Do You Offer?
We provide Veterans with:
- Meal Prep
- Bathing and Dressing
- Light House-keeping
- Laundry
- Medication Reminders
- Monitor their Safety
- Monitor Changes in Their Health Conditions
- Provide Participation in Home Exercise Programs (After proper training from a skilled clinician)
Do You Offer Short- and Long-Term Non-Medical Home Care Services?
Veterans are authorized an allotment of hours per week and the duration of approval is decided by the Veterans Affair Community Care Network.
Do You Provide Caregivers to Help Clients in Hospitals, Rehab Centers, and Assisted Living Facilities?
Our caregivers are non-skilled but can assist in assisted living facilities as well as independent living. Hospitals are skilled facilities.
Do You Come to the Home to Meet With the Client and/or Family Members?
Yes, we visit the client’s home for a thorough “In-Home Assessment” of the senior’s needs and the living situation. Our case manager then completes a “Plan of Care” that is helpful to our staff and the client’s family.
What Makes 1-on-1 Care Matters, LLC Different?
1-On-1 Care Matters, LLC is a home care agency pioneer in Montgomery County. Our company president has been in the therapy field for over 18 years with extensive experience in assisted living facilities. We are one of the few companies that offer a two-hour minimum per visit for Veterans. We also offer a choice of caregivers, a free in-home assessment of the Veterans' needs in the Kentucky and Tennessee communities.
Are Your Caregivers Supervised After They Are Placed With a Client?
Yes, after we make the initial placement, we are in constant touch with our caregiver and client to ensure that everything is working smoothly and that both parties are happy. Our case managers visit periodically (via phone and in person) to assess the situation in the home.
What Are Your Hours?
1 On-1 Care Matters caregivers are available from 2 to 24 hours a day. All services are available 7 days a week, including holidays. Our office hours are 8 A.M. to 5:30 P.M. Monday through Friday and 10 A.M. to 3 P.M. on Saturday. Of course, we are available 24 hours a day, 7 days a week by phone in case of emergency.
Who Are Your Caregivers?
We only recruit highly qualified caregivers. Most have worked previously in hospitals, rehab centers, or assisted living facilities.
How Are Your Caregivers Screened?
We thoroughly screen each prospective employee. Work history and personal references are checked. National background checks are conducted for criminal, DMV, and Social Security records. E-Verify, a national Homeland Security system, confirms names, dates of birth, and social security numbers. We also review and verify all professional credentials. All employees are TB and drug tested.
Can Your Caregivers Transport and Accompany Clients to Doctor’s Appointments and Activities Outside the Home?
We do not transport, but we can accompany the Veteran to their appointments or outings if there is a safe driver other than the client or the caregiver. Caregivers are able to ride on city transportation with Veterans for free.
What Other Services Do You Offer?
1-On-1 Care Matters, LLC is a great referral source for other services that help seniors and their families with their day-to-day living needs. Meals on Wheels, adult day care, assisted living facilities, Lifeline, transportation and medical supplies are just a few examples. Please call our office for more information.
How Does Your Service Work?
No two clients are alike. That is why we have created a four-step process to ensure a perfect fit.
Case Manager Home Visit: We schedule a convenient home visit to assess your loved one’s needs.
Plan of Care: We present a written plan of care. All hours, services, and fees are outlined clearly.
Caregiver Match: We recommend a choice of 2-3 caregivers. You choose the one that best matches your loved one’s needs.
Case Manager Visits: We visit periodically (via phone and in person) to personally assess your loved one’s case.
Will Insurance Companies Cover Non-medical Home Care?
Our services are paid from the Veterans Affairs Community Care Network there is no out-of-pocket expense for the Veteran.
Does the Veterans Administration Provide Veterans and Surviving Spouses With Assistance for Home Care?
Yes, if you are a Veteran or surviving spouse and you need assistance at home due to your disabilities, you may be eligible for a non-service disability pension. Please contact our office and speak to one of our case managers for more details.
Who Pays for the Services?
The Veterans Affairs Community Care Network pays for all services.
What Are the Minimum Hours You Require?
2 hours per visit.
Do You Provide Home Health Services?
Even though most of our caregivers at 1-On-1 Care Matters, LLC are not licensed medical professionals, they have a close working relationship with home health providers and can help you get in touch with medical personnel to assist you.
Do You Offer Hospice Services?
Even though most of our caregivers at 1-On-1 Care Matters, LLC are not authorized to provide hospice care to our clients, they have a close working relationship with hospice facilities and can help you make contact to ensure your loved one is comfortable in the last stages of life.
All of our caregivers have been trained on Dementia care and are skilled at taking care of client's with cognitive deficits. Most of them remain in the comfort of their homes until the end.
